This years flu vaccination clinic will be held at Worksop Bowling Club, on the following dates.

If you can't attend these clinic you can have the vaccination at the surgery simply book an appointment with one of our health care professionals.
Flu vaccination is recommended to patients ages 65 years and over, or patients who have Chronic Heart Disease, Chronic Respiratory Disease, including Asthma, Chronic Renal Disease, Chronic Neurological Disease, Diabetes, Immunosuppression due to disease or treatment, pregnant women and people who are the main cares for an elderly or disabled person whose welfare may be at risk if the carer falls ill.

If you haven't already got your username and password, please ask a member of the reception team.
We no longer require any proof of identity you just need to come to reception in person to collect your log on information.

This information (not your name) are sent to a secure system (HSCIC - Health and Social Care Information Centre) where it can be linked to other health information from different parts of the NHS in a way that does not identify you.
