Helpful facts for our online forms
- Patient Triage forms – submit ONE issue per form Only – or you will be asked to submit another form another day to ensure all patients have a fair chance to get an appointment.
- Our patient triage service is available online via our home page or via the NHS APP.
- Do Not use a form to request weight loss medication/jabs unless you meet the FULL criteria – see further down the page.
- URGENT MEDICAL ISSUE – A problem that needs attention within 24-48 hours. Once we have reached maximum safe working capacity for the day, we advise you contact NHS 111 or pharmacy for advice.
- If you do not have access to the internet or would struggle to complete a form online, then the receptionist will fill this out on your behalf over the phone.
- Please ensure you have your phone on loud for calls and SMS messages as you may receive an appointment or link via text that you will need to respond to.
- ROUTINE MEDICAL ISSUE
- You can submit a routine medical problem via our online patient triage forms as above between 8am and 6:30pm.
- You will then be sent a link or be directly contacted to book an appropriate appointment or will be asked to book an online appointment when they become available.
- ADMIN QUERIES – Sicknotes, Prescriptions, Results etc.
- We also have an online admin form for queries and prescriptions. This is open during our opening hours 8am-6:30pm and is for all non-urgent admin issues.
- All admin forms will be responded to within the working day. Therefore, if it is something that is urgent, please either submit an online MEDICAL form or call the practice in the usual way.
- For Test Results do not submit a Health Problem form – call 01909 500266 option 3 between the hours of 09.00 and 16.00.
- We appreciate your cooperation in this regard.
