Online Requests
Patient Triage forms – submit one issue per form only – or you will be asked to submit another form another day to ensure all patients have a fair chance to get an appointment.
Our patient triage service is available online via our home page or via the NHS APP.
For urgent medical problems:
- Fill out an online medical/health problem form
- A GP will review your form on the same day.
- You will be contacted by telephone or SMS with an appointment time or medical advice.
- You must be available to respond to the GP’s message.
If you cannot fill out the form, please telephone the surgery, and a receptionist can submit it for you.
For non-urgent requests (appointments, medication, administrative issues):
- Use the Admin or Routine Care Request form (to make a routine appointment please use the “Other admin” box).
- Important: This is not for urgent medical problems.
- Your request will be reviewed by our clinical or administrative team. Submissions are reviewed during our normal working hours, and we will provide a response once your request has been assessed. Response times can differ depending on the nature of your enquiry and overall demand.
Please note: this service is not monitored continuously, and response times may vary.
Communication – You may get a call or text from the practice, so it is important that you keep your phone close to hand and stay alert to incoming messages and calls.
